Important Information for Physicians

The disease management program, offered by LifeMasters, is designed to complement your plan of care and offer support to your patients over the phone.

Patients who participate in the program receive education about their disease, tools for monitoring their vital signs and symptoms, and support in developing self-care skills. Depending on their condition, patients may also receive regularly scheduled calls from nurses and health educators. The goal of this program is to reduce serious complications and unnecessary hospitalizations.
