Information about the LifeMasters program from Medicare
You may have been selected to participate in a program from Medicare designed to help you better manage your health. It is only offered to people with heart failure, or any combination of heart failure, diabetes and heart disease. If you have been selected, you may have received a packet of information or a phone call from LifeMasters.
Imagine that you could have your own personal nurse. Someone who understands your personal needs. Someone who would call you on a regular basis to check on you. Your nurse would:
- Listen and learn about what is important to you
- Help you set health goals that you feel you can do
- Support and encourage you in reaching your health goals
- Give you information about your health condition
- Answer your health questions and give you helpful ways to stay healthy
- Understand your life situation and how it may affect your health
- Help you follow your doctor’s plan
- Answer questions you wished you had asked your doctor
- Teach you how to track your progress
- Explain your doctor's treatment plan and recommendations
- Help you understand your medications
- Answer questions your family or caregivers may have
- Give you tips on healthy meal choices and food substitutions
- Care about YOU and be there when you need someone to talk to
You get to keep the same doctor and insurance you have now. And, it costs you nothing.
Wouldn't that be worth trying?
Medicare provides just that - your own personal nurse provided by LifeMasters. The program is an additional no cost Medicare service. Medicare is offering this program to you because you may have heart failure or any combination of heart failure, diabetes or heart disease. You have been selected to participate by Medicare to take advantage of the program. It is not offered to all Medicare or Medicaid beneficiaries in Florida. Together with LifeMasters, Medicare hopes to help you better manage your own health.
We understand you may be thinking this is too good to be true. There must be a catch. So we decided the best thing to do was to let others who have joined the program tell you about their experience:

As of March 5, 2007, the LifeMasters program is available in the following Florida counties: Alachua, Broward, Miami-Dade, Marion, Palm Beach, Seminole and Volusia.
