Welcome to the LifeMasters Program from Medicare for Florida residents
The Centers for Medicare & Medicaid Services (Medicare) and LifeMasters, a national leader in health care support, launched an important health support program in Florida* in 2005. The LifeMasters program is available to people who have both Medicare and Medicaid coverage and heart failure, or any combination of heart failure, diabetes and heart disease.
This program is available at no cost to eligible participants - those selected by Medicare. Their Medicare and Medicaid benefits will not change when they participate. Once in the program, participants are assigned a personal nurse who provides health support and education to help them better understand and follow their doctor's treatment plan, and learn how to better manage their health.
This program from Medicare is not an HMO or a health insurance plan. Program participants will continue to see their same doctor, receive the same benefits and their healthcare coverage will not change.
