Company Overview
LifeMasters Supported SelfCare, Inc. is an exciting national disease management company whose mission is to help individuals achieve and maintain optimal health by closing the gaps in medical care and encourage the adoption of healthy lifestyles. With more than a decade of experience, we are experts at delivering disease management services to individuals with chronic diseases such as diabetes, cardiovascular disease, respiratory disease, musculoskeletal conditions and their co-morbidities.

LifeMasters health professionals coach and educate participants over the phone, teach them how to monitor their vital signs and symptoms, and ultimately take more responsibility for their health. We involve the participant's physicians by providing them with relevant medical information between office visits to prevent unnecessary complications. This often results in participants establishing a better relationship with their physicians and can help them avoid unnecessary hospitalizations and emergency room visits. Our programs work and we have validated cost savings and improvement in clinical indicators through peer-reviewed studies.

We accomplish this by providing individuals who have chronic health problems and their physicians with the tools and information they need to better manage their health. As a result of our efforts, individuals stay healthy longer and their overall healthcare costs are lowered which is why our clients (health plans, governmental agencies, retirement systems and employers) hire us to provide programs for their populations.
