LifeMasters Contracts With Presbyterian Health Plan to Provide Disease Management Services

LifeMasters to Offer Monitoring and Counseling to Chronically Ill with Coronary Artery Disease (CAD) and Diabetes

Irvine, Calif. (April 9, 2003) - LifeMasters® Supported SelfCareSM, Inc. a leading disease management company, today announced it has signed an agreement to provide disease management services to Presbyterian Health Plan members suffering from coronary artery disease (CAD) and diabetes. Under the three-year contract, eligible members in the 300,000 member Presbyterian Health Plan began enrolling in LifeMasters' program as of April 1, 2003. Presbyterian Health Plan is the largest private health plan in New Mexico.

Once enrolled, Presbyterian's commercial, Medicare+Choice and Medicaid members with CAD or diabetes will benefit from LifeMasters' remote monitoring and coaching. Enrolled high-risk members will transmit vital sign information from their home daily to a LifeMasters nurse, who watches for irregularities and will immediately contact the beneficiary and alert his or her physician if there is a problem. Nurses will also coach Presbyterian's eligible members on proper diet, exercise and other healthy behaviors-keeping them healthier and reducing unnecessary costs such as expensive trips to the emergency room or surgeries.
