LifeMasters Contracts with Prevea Health Plan
to Provide Disease Management Services

LifeMasters to Monitor and Coach Health Plan Members with Diabetes, Asthma, and Coronary Artery Disease

Irvine, Calif. (July 13, 2004) - LifeMasters® Supported SelfCare^SM, Inc., a leading disease management company, today announced it has signed an agreement to provide disease management services to Prevea Health Plan (PHP) members with diabetes, asthma, and coronary artery disease (CAD). Prevea Health Plan offers customizable health insurance plans to northeastern Wisconsin employers in six county service areas including Brown, Door, Oconto, Kewaunee, Manitowoc and Outagamie counties.

Under the three year contract, members enrolled in Prevea Health Plan's commercial HMO and POS plans will have access to disease management services, including customized health education, lifestyle-modification support, regular telephonic interaction with a clinical nurse consultant and regular monitoring of the member's vital signs and symptoms for early identification of changes in health status. In addition, LifeMasters communicates with the member's physician whenever a clinical problem arises.
