An Unparalleled Primary Nurse Model
At LifeMasters, we believe that developing a personal relationship with each participant is vital to the success of the program. That’s why each time a participant has an interaction with LifeMasters, it is with the same nurse—someone who is familiar with their complete health history and their life situation.

LifeMasters nurses are highly-trained health professionals who serve as personal coaches, educators, motivators and health advisors. They create a connection between the patient and their physician, ensuring that patients comply with their physician’s treatment plan. The LifeMasters nurse trains and encourages patients in a variety of ways. Participants are educated about their chronic illness(es) and co-morbidities and taught how to track important vital signs and symptoms to detect changes in their health status early enough to avoid complications and possible hospitalizations. They receive training on how to make better food choices and start a suitable exercise program. They are also encouraged to take their medications regularly and learn how to avoid situations that might worsen their condition. Nurses offer ongoing support and motivation to participants and their caregivers to make the necessary lifestyle changes to improve their health.

“Wanda and I have been working together to improve her health for about 2 years.  When she first started the program her blood glucose was out of control and her HbA1c was elevated. We had long conversations about possible kidney damage and discussed lifestyle changes.  Over time she lowered her HbA1c significantly by watching her carbohydrate intake.  She credits the success to our conversations and an article in Living Well, LifeMasters’ health education magazine.  To this day she still refers to it.  Wanda now feels empowered and collects all of her lab results so that she can see her achievements."
