The LifeMasters Program

How the Program Works

Your doctor's care is important. But so is learning as much as you can about your symptoms or condition, and taking steps to stay well. To get started, we call you to learn about your health history. Then, you talk about your needs with a LifeMasters health professional and decide together what kind of program is best for you. This may mean we call you regularly, keep track of your vital signs and symptoms and send you helpful health information.

We do not replace your relationship with your doctor, but in some instances we keep your doctor up to date about changes in your health. We can also teach you how to monitor your health, including measuring and recording your vital signs and symptoms. Sharing this information with your doctor can improve the care you receive and make you an active partner in managing your health. We understand that no two people are alike. That is why we personalize our program to give you just what you need. A better quality of life is possible when you are a partner in managing your own health.

You may be eligible for the LifeMasters program, which is offered to many people throughout the United States. Currently over 600,000 people participate in the LifeMasters program, however the program is only available through your health insurance. Contact your health coverage company or your benefits administrator at your employer to see if the LifeMasters program is available.
